           Behavioral Health Integration in a Patient-Centered Medical Home (PCMH)
           Katherine K. Plunkett, LMSW, MPH, CHES, CPHIMS

1. Discuss why behavioral health integration is needed in a patient-centered medical home
2. Define the levels of behavioral health integration found in a patient-centered medical home.
3. Discuss how behavioral health integration is included into PCMH Recognition standards.
4. List measures that evaluate behavioral health integration into a PCMH.

Advance Care Planning for Professionals
Amanda Werts, LMSW, MSW, BSW

1. Identify advance directives utilized in S.C.
2. Identify the problem of limited completion of advance directives
3. Understand the inherent problem with our current approaches to advance care planning.
4. Identify better advance care planning approaches

Dementia Speaks, Don’t mind me!
Macie Smith, Ed.D, LBSW, C-SWCM, SW-G

1. Participants will become better informed about effectively managing the care of those living with dementia.

CREDIT:

AHEC: This program is approved for 0.9 CEU’s (9.0 clock hours) of continuing education by Mid-Carolina AHEC, Inc. and meets the SC AHEC Best Practice Standards.  

SOCIAL WORKERS: This program has been approved by the SC Board of Social Work Examiners for 9.0 clock hours of social work continuing education.

LONG TERM CARE ADMINISTRATORS: This program is pending approval by the South Carolina Board of Long Term Care Administrators for 9.0 hours.  (Approval #2018205).

Participants must attend 90% of the program in order to receive a certificate of attendance.  No partial credit will be given.
